Are You at Risk for Heart Disease?

Find out with the Cedars-Sinai Cardiac Risk Test. This anonymous questionnaire will ask about lifestyle and health habits that affect your heart health. You'll receive an instant assessment of your risk for heart disease: low, moderate or high.

8. If you know your blood pressure, enter the numbers here. If you do not know your actual blood pressure, has a doctor or nurse told you that it was high, normal or low?

10. What is your HDL cholesterol (the "good" cholesterol)? If you do not know your actual HDL cholesterol level, has a doctor or nurse told you that it was high, normal or low?
