Welcome and thank you for choosing the Minimally Invasive Urology Institute at Cedars-Sinai Medical Center.

Whether preparing for your first visit or for a follow-up exam, our goal is to provide you with state-of-the-art clinical care with the highest possible outcomes. In order to facilitate a smooth registration process, we ask that you complete some forms that are needed by our Patient Care Team to develop a plan of care for you. Please print out the following forms*, complete them and bring them with you on the day of your visit:

We would like to remind you that you are being treated in a hospital outpatient facility - a part of Cedars-Sinai Medical Center. Billing procedures for hospital services differ from those submitted by private practice physicians. To help you understand how the hospital will bill for your visit today, please be sure to print out and read the "Billing Information" sheet, and feel free to direct any questions you may have to our billing supervisor.

If you are being seen for a new condition, please be sure to bring your most recent X-ray films (the actual films in addition to the printed report) and medical records related to your current medical condition. This information is vital in order to conduct a complete and thorough consultation.
