Returning Volunteer Information
Are you ready to return as a volunteer in our Academic 2008-2008 Teen Session? We're excited to have you back with us, but some of you may need to update your health clearance PRIOR to your first day as a volunteer. By mid-Sept., you should all receive a reply form inquiring about your academic participation. It will also direct you here so you can work on completing your necessary requirements.
If you are due for an updated health clearance, please download the following forms:
- *Instructions for How to Update your Health Clearance (rev)
- *Teen Health Forms Attachments 3-4 (rev. 2)
- *TB Test from Outside MD Attachment #6 (rev.)
- *Map to Employeee Health Services (rev.)
Once you have your health clearance, please bring it to the Volunteer Services Department.
