The Louis Warschaw Prostate Cancer Center is dedicated to the advancement of the treatment of prostate cancer. We firmly believe that empowering the patient through education regarding their healthcare decisions is fundamental.
At your initial appointment, the following forms (listed below) will be presented to you for review, completion and signature. Please read them carefully as they are comprehensive. Due to Institutional Practices, the original signed forms must be kept in your medical record chart. We will be unable to accept any pre-completed forms.
