If you are currently enrolled or would like to participate in the Cedars-Sinai Physician and Service Referral Program via 1-800-CEDARS-1, you must complete the new enrollment form. Participation is completely voluntary, so you must complete an enrollment form to receive referrals through this service.

Cedars-Sinai operates the Physician and Service Referral Program for calls received via 1-800-CEDARS-1, 24 hours a day, seven days a week. There are no charges to the physicians or to the callers for this service. Referrals are given on a rotating basis, but the final selection of a physician is based on the caller's needs and preferences. Cedars-Sinai medical staff members must be of the Attending or the Associate membership category to participate in this program.
