Include INDICATIONS in Order "Per Pharmacy" When writing "per pharmacy protocol" orders, the Pharmacy Dept. asks that physicians please remember to include the indication for the requested medication (e.g. vancomycin for osteomyelitis, warfarin for atrial fibrillation, etc.).
